The Nurse Case Manager, BHACC (CM) works face to face with the member, the members family and providers involved in the members care to coordinate the services necessary to help reduce preventable hospitalizations. The CM is responsible for developing a plan of care for the member in conjunction with BHACC clinicians, and the members PCP and specialist.
DUTIES AND RESPONSIBILITIES:
Provides face-to-face and telephonic Case Management to assigned members Develops an individualized plan of care and implements the plan Communicates with nursing staff, physicians, nurse practitioners and other health workers involved in the care of a member Coordinates with member, family, and caregivers to help resolve barriers to care Evaluates member/family strengths, health behaviors, and resources Intervenes with at-risk members to avoid unnecessary hospitalizations Successfully collaborates with member and family to assure comprehension of available resources Considers member/family physical, educational, and financial resources Provides on-call services, and travels between BHACC facilities as required Other duties as required
Requirements:
MINIMUM QUALIFICATIONS:
Registered Nurse with 3-5 years related experience and/or training in Case Management, or Licensed Practical Nurse with 5-7 years related experience and/or training in Case Management Social work training, licensure is highly desirable Medicare experience Experience with geriatric populations and community resource organizations Experience handling confidential health care information Experience with care management software applications Experience with / exposure to the case management of transplant candidates and recipients Excellent interpersonal and communications skills Ability to work within Microsoft Office applications (Excel, PowerPoint, Word, Access, and Outlook) Ability to meet deadlines and manage multiple priorities and work in a dynamic, fast-paced team environment Spanish language ability is desirable
