CLINICAL STROKE COORDINATOR
SUMMARY:
The Clinical Stroke Coordinator is responsible for: The education of stroke patient and to assure quality patient care. Coordinating the efforts of the hospital in order to meet the needs of the patients and the customers. Increase the quality of patient care by educational and QI programs. Providing expert knowledge, leadership and education for individuals within the hospital for stroke. Developing educational programs to enhance the staff's knowledge of current trends in nursing. Develop, implement and maintain a quality improvement process for the stroke patients in our hospital. Performing chart audits for quality improvement of documentation and risk reduction, in order to maintain and/or improve patient outcomes. Collaborating and assisting in developing and implementing projects that enhances the quality of the hospital for stroke.
POPULATION SERVED:
The Stroke/Cerebrovascular Clinical Coordinator facilitates the bedside clinical care of Stroke/TIA, and other complex cerebrovascular patients to assure that all patients receive comprehensive and timely treatment interventions that are consistent with The Medical Center defined Clinical Practice Guidelines. This position collaborates with all members of the healthcare team via consistent clinical patient rounds to assist in the planning, implementing, monitoring and evaluation of care delivery for the stroke/TIA/complex cerebrovascular patients. The Stroke/Cerebrovascular Clinical Coordinator provides formal and informal education to staff and community related to cerebrovascular-specific topics, and acts as a resource for staff, physicians, and community to support Stroke/cerebrovascular program goals. The Stroke/cerebrovascular Clinical Coordinator position collaborates with nursing and physician leadership and the Education Department to facilitate cerebrovascular-related research projects.
Core competencies will be assessed primarily on the following patient population(s) served: Acute medical/surgical care of diseases and conditions including but not limited to: ischemic stroke patients and Transient Ischemic Attack patients
ESSENTIAL FUNCTIONS:
1. Demonstrates knowledge and application of the nursing process for Neuroscience patients of appropriate ages in each assigned areas; i.e. ischemic stoke classification.
2. Coordinator of the Stroke Program will be responsible for the management and implementation of Core Measure protocols, Nursing PI and data collection processes. He/She will educate nursing staff and medical personnel to achieve quality initiatives. The Stroke Program Coordinator will work with the nursing staff, managers and directors of all ancillary departments in establishing and implementing and monitoring hospital wide protocols/processes for the purpose of developing standardized structure and quality assurance. He/She will be responsible for reporting all Nursing PI to Quality Council and other hospital committees. Will serve as a resource and educator for the nursing staff, including other ancillary departments
3. Assesses and monitors and recognizes changes in the patient's behavioral, neurological and physiological status; takes appropriate action based on assessment.
4. Utilizes professional skills and judgment in delivering and/or delegating care for patients.
5. Assists with unit employee process activities, including orientation, professional development.
6. Provides for the processing of patients transitions thru department, and movement of patients to other levels of care; i.e. rehab.
7. Assists in the Performance Improvement activities of the department, such as stroke, pain, adaptation, mobility, and outcomes.
8. Educates the patient/family based on individual needs. Will need 10 hours of stroke education per year.
9. Provides and ensures patient safety at all times
10. Specializes in the care of patient's with biopsychosocial alteration due to nervous system dysfunction.
11. Responsible for maintaining and meeting AANN and JCAHO standards.
12. Other duties as assigned within scope and practice of licensure.
Qualifications
REQUIRED LICENSURE AND/OR CERTIFICATION
1. BCLS
REQUIRED EDUCATION:
1. RN licensed in State of Texas or compact State Nursing license
REQUIRED EXPERIENCE:
1. ICU experience
PREFERRED EDUCATION, LICENSURE, CERTIFCATION, EXPERIENCE, SKILLS, KNOWLEDGE AND ABILITIES:
1. Preferred experience as RN in Critical Care
2. Effective communication skills; both orally and written.
3. Computer skills; i.e. word processing systems
4. Ability to work with others within a team to ensure quality patient care.
5. Strong problem solving skills (Critical Thinking Skills).
6. Ability to move an excess of 100 pounds, with assistance from other staff.
7. Knowledge of general hospital operations
