Transplant Coordinator

Summary:

The transplant coordinator will be solely responsible for managing and leading in-patient rounds, working with the in-patient teams to ensure goals of care are met, provide real time mentoring and education to in-patient teams and will ensure smooth, safe transitions of care.

Under the general direction of the nursing and physician leadership, the transplant nurse coordinator is responsible for providing care, education, and support to potential transplant candidates, potential transplant donors, patients active on the transplant waiting list, and post-transplant patients both in the hospital and in the community. A transplant nurse coordinator collaborates in the delivery of health care services with other team members including but not limited to pharmacy, social work, nutrition, finance, radiology, physical therapy, laboratory, and case management. A transplant nurse coordinator interacts with patients in the clinic, performs nursing assessments, and consults with other care providers in developing treatment plans. A transplant coordinator will have in-patient nursing responsibilities including but not limited to, facilitating multidisciplinary team rounding, developing and managing the goals of care, patient education, and facilitating discharge. A transplant nurse coordinator provides telephone triage for pre and post-transplant patients and live donors and coordinates care for pre-and post-transplant patients, providing professional case management of organ transplant-related activities. Patient, family, live donor, and nursing colleague education is an integral part of the transplant nurse coordinator's role.

The transplant nurse coordinator, in collaboration with the transplant program and in-patient leadership, identifies learning needs and facilitates the provision of provider and nursing education, including outreach education to dialysis centers and nephrology offices.

Requirements:

1) Partners with identified medical leaders to develop and implement expert clinical care delivery systems for a defined patient population or service. Utilizes the nursing process to promote quality, safe and attentive patient care.

2) Demonstrates outstanding clinical and service delivery and utilizes critical thinking, problem-solving, and expert knowledge to identify and facilitate systems improvements with a strong focus on health promotion, risk reduction, and population-based health care.

3) Assumes leadership role in planning and implementing clinical initiatives and quality improvements that demonstrate effective positive progress in key performance metrics such as patient satisfaction, LOS, cost/case, and staff engagement or others as defined by program goals

4) Collaborates with inter-professional team members to enhance outcomes across the continuum of care by utilizing, critiquing, and/or conducting research to promote evidenced based practice, including publication in professional journals and poster abstracts/presentations.

5) Identifies high-risk patient care issues and trends and implements interventions to achieve optimal/benchmark quality outcomes. Participates in program process improvement activities and is familiar with unit-based measurement data. Works with the program to improve the quality and cost-effectiveness of patient care.

6) Assumes leadership role in population-based clinical strategic planning and participates on key committees related to that planning.

7) Establishes collaborative relationships with regional providers (primary care, post-acute) and local community organizations to advance population health across the region.

8) Develops and provides clinical education to direct care, clinical staff, cross-continuum inter-professional team members as well as patients and families.

9) Participates in community education and awareness campaigns to advance population-based health initiatives.

10) Knowledgeable about health care/payment reform and its impact on a given population. Participates in creating novel delivery models to meet changing regulations and mandates.

11) Develops and/or utilizes appropriate teaching tools for recipients, families, and nursing colleagues.

12) Collaborates with nursing and physician leadership in establishing policies and procedures for the transplant program.

13) Maintains a high level of professional standards by participating in continuing education, professional organizations, and community outreach.

14) Delegates work appropriately and provide direction and guidance to nursing and support staff. Demonstrates effective communication skills throughout the continuum of patient care.

15) Respects the values (religious/spiritual, ethnic, cultural) of the patient and family.

16) Documents relevant clinical issues in the patient's medical records.

17) Adheres to required transplant regulations as put forward by OPTN and CMS

Required Experience:

3 years of relevant nursing experience

Required Education:

Bachelor's Degree

Preferred Experience:

5 years of relevant nursing experience

Experience in Renal, Dialysis, and/or Transplant

Bi-Lingual

Preferred Education:

Master's degree in CNL, MSM MS, NP
